What to expect when visiting the Diabetes Education Department at Windham Hospital.

The Diabetes Education Program at Windham Hospital has held a “Certificate of Recognition” from the American Diabetes Association since 1991. Windham Hospital was the first community hospital in the United States to meet the National Standards for Diabetes Self-Management Education and to be awarded this Recognition status by the American Diabetes Association. 
The individuals who provide you with education through this program have specialized training and include a Registered Nurse Certified Diabetes Care and Education Specialist (RN CDCES) and a Certified Dietitian Nutritionist/Registered Dietitian (CDN/RD).   

We offer an individualized approach to education and training on diabetes self-management. Once you have been referred by your physician for Diabetes Education and/or Diabetes Medical Nutrition Therapy, our Access Center scheduling team will contact you to schedule your first appointment. All education sessions are one‑on‑one and held in person. Your initial visit lasts 60 minutes, and follow‑up sessions are 30–60 minutes based on your individualized education plan.

Virtual appointment sessions may be available depending on your insurance coverage.
